Youth Transition Services has been established to support young people (15-19 years) to undertake activities that contribute to their long term economic independence and well being. The goal is to keep them engaged in further education, training or employment. 

A team of coaches and mentors will work with young people to establish an individualised pathway plan, and support them on their journey, maintaining regular contact. The service will also help young people with health, financial and other social service advice.  Referrals will be made to appropriate education and training providers, and other social services as required.
