The Pāhake Whaiora Primary Care service works alongside pāhake (older people) who are experiencing early stage dementia, and age related conditions, including mental health and addiction problems. This community-based service is available for whānau throughout Taranaki. The service can work with community providers to:
Navigate pathways for older persons in community and hospital settings
- Identify ‘what’s needed’
- Liaise and attend specialist care appointments
- Demystify the health speak
Support pāhake and their whānau in this stage of their lives
- Keep whānau informed of ‘what’s available’
- Maintain connection to their community and natural supports
Whānau Ora based interventions that help:
- Pāhake and whānau determine ‘what’s best’
- Include cultural support from Kaumātua/Kuia
