This service assesses and coordinates the care of adults aged between 18-65 who require community based, supported accommodation and/or respite services. It is available to clients from within mental health and addiction services. Our team works alongside the client and their whānau to support recovery and rehabilitation skills and assess and coordinate community care and support.
