About this Service

This service assesses and coordinates the care of adults aged between 18-65 who require community-based, supported accommodation and/or respite services. It is available to clients from within mental health and addiction services.

Our team works alongside the client and their whānau to support recovery and rehabilitation skills and assess and coordinate community care and support.

Who is this service for?

People 18 years of age and older, receiving services from Mental Health Services can be referred to the Needs Assessment and Service Coordination Service.
