The Pāhake Whaiora Primary Care service works alongside pāhake (older people) who are experiencing early-stage dementia, and age-related conditions, including mental health and addiction problems. This community-based service is available for whānau throughout Taranaki. The service can work with community providers to:
Navigate pathways for older persons in community and hospital settings
Support pāhake and their whānau in this stage of their lives
Whānau Ora based interventions that help:
People 55 years and older with mild to moderate mental illness or dementia.This service is run in partnership with the Taranaki District Health Board.
