About this Service

The Pāhake Whaiora Primary Care service works alongside pāhake (older people) who are experiencing early-stage dementia, and age-related conditions, including mental health and addiction problems. This community-based service is available for whānau throughout Taranaki. The service can work with community providers to:

Navigate pathways for older persons in community and hospital settings

  • Identify ‘what’s needed’
  • Liaise and attend specialist care appointments
  • Demystify the health speak

Support pāhake and their whānau in this stage of their lives

  • Keep whānau informed of ‘what’s available’
  • Maintain connection to their community and natural supports

Whānau Ora based interventions that help:

  • Pāhake and whānau determine ‘what’s best’
  • Includes cultural support from Kaumātua/Kuia

 

Who is this service for?

People 55 years and older with mild to moderate mental illness or dementia.This service is run in partnership with the Taranaki District Health Board.
